Kistik Ekinokokkozis Tanısında IgE Ve IgG Cevaplarının Korelasyonu

AMAÇ: Bu çalışma kistik ekinokokkozis (KE) ön tanısı alan alerjik semptomlu hastalarda serolojik tanı yöntemlerinin korelasyonunu araştırmak amacıyla yapılmıştır.
YÖNTEMLER: Alerjik semptomları olan fzik muayene ve radyolojik incelemeler sonucunda KE ön tanısı alan 48 hastaya ait serum örnekleri toplanmıştır. Bunlarda KE IgG antikorları ELISA (Enzyme Linked Immunosorbent Assay) ve IHA (Indirekt Hemagglütinasyon) testleriyle araştırılmıştır. Ayrıca Echinococcus granulosus’a özgül spesifk IgE, total IgE ve eosinoflik katyonik proteinler de (EKP) ELISA ile değerlendirilmiştir.
BULGULAR: ELISA ve IHA sonuçları birlikte değerlendirildiğinde 24 olguda (%50) anti-E. granulosus IgG antikorları saptanmıştır. Spesifk IgE cevabı ile ELISA (r=0,90;p<0,001) ve IHA (r=0,89;p<0,001) sonuçları arasında güçlü bir korelasyon saptanmıştır. Spesifk IgE cevabının KE’ yi saptamadaki duyarlılığı %95,8; özgüllüğü %91,7; pozitif prediktif değeri %92 ve negatif prediktif değeri %95,6 olarak bulunmuştur. Pozitif olguların EKP sonuçları da negatif olgulardan anlamlı bir şekilde yüksektir (p=0,03).
SONUÇ: KE’de spesifk IgE yanıtı ile IgG yanıtı arasında güçlü bir korelasyon saptanmış ve hastalığın tanısında IgE’nin de kullanılabileceği düşünülmüştür.

The Correlation of IgE and IgG Responses in Diagnosis of Cystic Echinococcosis

OBJECTIVE: It was aimed to evaluate the correlation of serological diagnostic methods among patients with allergic symptoms prediagnosed as cyst hydatid diseases and search for different antibodies.

METHODS: We applied ELISA and IHA tests to 48 patients who had allergic symptoms and preliminary diagnosis of cystic echinococcosis as a result of physical examination and radiological evaluations. The IgG antibody levels are determined by ELISA and IHA. Furthermore, Echinococcus granulosus specifc IgE, total Ig E levels and ECP(eosinolic cathionic protein) were also evaluated using ELISA method.
RESULTS: When both ELISA and IHA results were evaluated 24 cases (50%) were determined with anti-E.granulosus antibodies. It was found a strong correlation between specifc IgE response with the results of ELISA (r=0,90;p<0,001) and IHA (r=0,89;p<0,001). Sensitivity, specifcity, positive predictive value and negative predictive value were 95,8%, 91,7%, 92% and 95,6% respectively in specifc IgE positive patients. Positive cases ECP results are higher than negative cases (p=0,03).

CONCLUSION: Strong correlation between specifc IgE and IgG responses was determined and it is postulated that IgE could be used in the diagnosis of cystic echinococcosis.
