HBsAg, Anti-HBs, Anti-HCV and Anti-HIV seroprevalence among patients admitted to our hospital; five-year retrospective data

INTRODUCTION: Viral hepatitis and human immunodeficiency virus (HIV) infections are important health problems in our country as well as all over the world. We aimed to investigate the distribution of seropositivity of HBsAg, anti-HBs, anti-HCV and anti-HIV according to years, genders and age groups in patients who applied to our hospital.
METHODS: Results of HBsAg, anti-HBs, anti-HCV and anti-HIV tests performed during 2015-2019 in our hospital were reviewed retrospectively. 77562 serum samples were studied for HBsAg, 45045 for anti-HBs, 73223 for anti-HCV and 71061 for anti- HIV of a total of 266891 samples. The tests were analysed using chemiluminescent microparticle immunoassay (Architect i 1000, Abbott, USA) and electro-chemiluminescence immunoassay (Cobas 6000,Roche Diagnostic GmbH, Germany) methods. Data were collected from the database of our hospital. Data were analyzed using the SPSS software ver. 24.0. Chi-Square (χ2) analysis and Fisher's Exact test were used for comparison of categorical variables. The p< 0.05 was considered statistically significant.
RESULTS: HBsAg, antiHBs, anti-HCV and anti-HIV positivity were 2.3 % (1808/77562), 46.6% (20975/45045), 0.6% (456/73223) and 0.05% (35/71061) respectively. Of the 1808 patients were positive for HBsAg, 55% were males, 45% were females. Of the 456 anti-HCV positive patients, 63.6% were females and 36.4% were males. When assessed by age groups, HBsAg positivity was found to be the highest in the age group 31-50 (38.1%) and lowest in the age group ≤ 18 (0.6%). Anti-HCV positivity was found to be the highest in the age group ≥65 (32%) and lowest in the age group ≤ 18 (2%). All patients who were found to be anti-HIV positive were male and the highest positivity rate was between the ages of 19-30.
DISCUSSION AND CONCLUSION: Our HBsAg, anti-HBS, anti-HCV and anti-HIV results were found to be similar with the country statistical data. There was significant difference in the seropositivity of HBsAg (p<0,001), anti-HCV (p<0,05) and anti-HIV (p<0,001) during the measurement period.

Hastanemize başvuran hastalarda HBsAg, Anti-HBs, Anti-HCV ve Anti-HIV seroprevalansı: 5 yıllık retrospektif veri

GİRİŞ ve AMAÇ: Viral hepatitler ve insan immün yetmezlik virüsü (HIV) enfeksiyonu tüm dünyada olduğu gibi ülkemizde de önemli sağlık sorunlarındandır. Çalışmamızda hastanemize başvuran hastalarda HBsAg, anti-HBs, anti-HCV ve anti-HIV seropozitifliğinin yıllara, cinsiyetlere ve yaş gruplarına göre dağılımını araştırmayı amaçladık.
YÖNTEM ve GEREÇLER: 1 Ocak 2015 ve 31 Aralık 2019 yılları arsında hastanemize başvuran hastaların, HBsAg, anti-HBs, anti-HCV ve anti-HIV test sonuçları retrospektif olarak incelenmiştir. Toplam 266891 serum örneğinin 77562'sinde HBsAg, 45045'inde anti-HBs, 73223'ünde anti-HCV ve 71061'inde anti-HIV testleri, kemilüminesans mikropartikül immunoassay (Architect i2000, Abbott, USA) ve elektrokemilüminesans immünoassay (Cobas e601, Roche Diagnostic GmbH, Germany) yöntemleri ile çalışılmıştır. Veriler hastanemizin veri tabanından elde edilmiş olup istatistiksel analizinde IBM SPSS 24.0 programı kullanılmıştır. Kategorik değişkenlerin analizinde Ki-Kare (χ2) testi ve Fischer’s Exact testi kullanılmıştır. İstatistiksel olarak p<0.05 anlamlı kabul edilmiştir.
BULGULAR: Çalışmamızda HBsAg %2.3 (1808/77562), anti-HBs %46.6 (20975/45045), anti-HCV %0.6 (456/73223) ve anti-HIV %0.05 (35/71061) oranında pozitif saptanmıştır. HBsAg pozitif saptanan 1808 hastanın %55'inin erkek, %45'inin kadın olduğu görülmüştür. Her iki cinsiyette de en fazla pozitifliğin 31-50 yaş aralığında (%38.1), en düşük ise ≤ 18 yaş grubunda olduğu (%0.6) saptanmıştır. Anti-HCV pozitif saptanan 456 hastanın %63.6'sının kadın, %36.4'ünün erkek olduğu görülmüştür. Anti-HCV testinde en fazla pozitifliğin ≥65 yaş grubunda (%32) olduğu görülürken, en düşük pozitiflik oranı ise yine ≤ 18 (%2) yaş grubundadır. Anti-HIV pozitif bulunan hastalar incelendiğinde ise pozitif bulunan tüm hastaların erkek olduğu ve en fazla pozitiflik oranının 19-30 yaş aralığında olduğu görülmüştür.
TARTIŞMA ve SONUÇ: Çalışmamızda tespit etiğimiz HBsAg, anti-HBs, anti-HCV ve anti-HIV seropozitiflik oranlarının ülke verileri ile uyumlu olduğu görülmüştür. HBsAg (p<0,001), anti-HCV (p<0,05) ve anti-HIV (p<0,001) seropozitiflik oranlarında yıllara göre istatistiksel olarak anlamlı farklılık olduğu saptanmıştır.
