
GİRİŞ ve AMAÇ: Gram-negatif, mikroaerofil bir bakteri olan Campylobacter, dünya genelinde gıda kaynaklı gastroenteritlerin önemli bir etkenidir, artan enfeksiyon sıklığı ve antimikrobiyal direnç nedeniyle halk sağlığı için ciddi bir tehdit oluşturmaktadır. İnsan ve hayvan enfeksiyonlarında florokinolonların yaygın kullanımı, çoklu ilaca dirençli Campylobacter suşlarında artışa yol açmıştır. Makrolidler, bu enfeksiyonların tedavisinde kritik öneme sahiptir ve mikroorganizmanın makrolidlere direnç durumunu bilmek etkili tedavi stratejileri geliştirmek için hayati önem taşır. Bu çalışmada, hastanemizde izole edilen Campylobacter suşlarının antibiyotiklere duyarlılıklarının belirlenmesi, özellikle makrolid direncinin fenotipik ve genotipik yöntemlerle değerlendirilmesi amaçlanmıştır.
YÖNTEM ve GEREÇLER: Çalışmaya, laboratuvarımızda son bir yıl içinde klinik örneklerden izole edilen ve MALDI-TOF MS ile tanımlanan toplam 47 Campylobacter türü dahil edilmiştir. İzolatların siprofloksasin, tetrasiklin ve eritromisine duyarlılıkları, EUCAST kılavuzları versiyon 14.0’a göre disk difüzyon testi ile belirlenmiştir. Eritromisine dirençli izolatlardan DNA elde edilmiş özgün primerler kullanılarak 23S rRNA ile L4 ve L22 ribozomal proteinlerini kodlayan gen bölgeleri PZR ile çoğaltılmıştır. Sanger yöntemiyle dizilenen ürünler NCBI-Blast ile analiz edilmiştir.
BULGULAR: Toplam 47 izolatın 43 (%91,5)’ü dışkıdan, 1 (%2,1)’i kandan ve 3 (%6,4)’ü apseden izole edilmiştir. İzolatların 41 (%87,2)’i Campylobacter jejuni, 5 (%10,6)’i Campylobacter coli ve 1 (%2,1)’i Campylobacter sputorum ssp. bubulus olarak tanımlanmıştır. İzolatların çoğu (n=39, %82,9) 0-18 yaş arası hastalardan izole edilmiştir. C. jejuni izolatlarında siprofloksasin, tetrasiklin ve eritromisine direnç oranları sırasıyla %90,0 %78,0 ve %4,8 olarak belirlenmiştir. Beş C. coli izolatından dördü siprofloksasine, dördü tetrasikline ve iki tanesi eritromisine dirençli saptanmıştır. İki yaşındaki bir çocuktan izole edilen bir C. coli suşu, test edilen üç antibiyotiğe de dirençli iken, C. sputorum ssp. bubulus test edilen tüm antibiyotiklere duyarlı çıkmıştır. Makrolid direnci gösteren dört izolatın üçünde 23S rRNA bölgesinde A2075G ve C2113T mutasyonları saptanmıştır. Ayrıca, L22 proteininin C-terminal ucunda çeşitli delesyon, insersiyon ve amino asit değişimleri gözlenirken, L4 proteinlerinde varyasyon tespit edilmemiş, ermB geni amplifiye edilmemiştir.
TARTIŞMA ve SONUÇ: Her iki türde de siprofloksasin ve tetrasikline karşı yüksek direnç oranları gözlenmiştir. Eritromisin direnci düşük olsa da yakından izlenmesi gerekmektedir.

INTRODUCTION: Campylobacter, a Gram-negative, microaerophilic bacterium, is an important cause of foodborne gastroenteritis worldwide and poses a serious threat to public health due to the increasing frequency of infection and antimicrobial resistance. The widespread use of fluoroquinolones in human and animal infections has led to a rise in the incidence of multidrug-resistant Campylobacter strains. Macrolides are critical antibiotics in the treatment of these challenging infections, and understanding the resistance profiles is vital for developing effective treatment strategies. This study aimed to determine the antibiotic susceptibilities of Campylobacter strains isolated in our hospital, and especially to evaluate macrolide resistance by phenotypic and genotypic methods.
METHODS: A total of 47 Campylobacter spp. isolated from clinical specimens in our laboratory over the past year, identified by MALDI-TOF MS, were included in this study. Disk diffusion testing was performed to determine the susceptibility of these isolates to ciprofloxacin, tetracycline, and erythromycin, according to EUCAST guidelines version 14.0. The DNA was extracted from erythromycin-resistant isolates, gene regions encoding 23S rRNA and L4 and L22 ribosomal proteins were amplified using specific primers by PCR and the PCR products sequenced by the Sanger method were analysed with NCBI-Blast.
RESULTS: Of the 47 isolates, 43 (91,5%) were isolated from stool, 1 (2.1) from blood, and 3 (6,4%) from abscesses. 41 (87,2%) isolates were identified as Campylobacter jejuni, 5 (10,6%) as Campylobacter coli, and 1 (2,1) as Campylobacter sputorum ssp. bubulus. The majority of isolates (n=39, 82,9%) were recovered from patients aged 0-18 years. Resistance rates in C. jejuni to ciprofloxacin, tetracycline and erythromycin were 90,0%, 78,0% and 4.8%, respectively. C. coli isolates, 4 of 5 resisted ciprofloxacin and tetracycline, and 2 of 5 resisted erythromycin. Notably, one C. coli from a 2-year-old child was resistant to all three tested antibiotics, while C. sputorum ssp. bubulus remained fully susceptible. Mutations A2075G and C2113T were detected in the 23S rRNA region in three of the four macrolide-resistant isolates. Additionally, various deletions, insertions and amino acid changes were observed in the C-terminal end of the L22 protein, but no variations were found in the L4 proteins and the ermB gene not detected.
DISCUSSION AND CONCLUSION: High resistance rates to ciprofloxacin and tetracycline were observed in both species. Erythromycin resistance, although low, is worrisome and should be closely monitored.
